The Community Health Improvement Plan (CHIP) is Concord’s developing roadmap for addressing local public health priorities. Informed by the results of the 2024 Community Health Needs Assessment (CHNA), the CHIP is being created in collaboration with residents, local organizations, and members of the Concord Behavioral Health Collaborative.
The CHIP will outline shared goals, strategies, and actions to improve the health and well-being of the community. Early planning has identified mental health, substance use, and isolation and loneliness as key areas of focus. The plan will also include:

Lead organizations and partners for implementation
Potential funding opportunities
A timeline for action and ongoing progress review
