In 2024, the Concord Health Division conducted a Community Health Needs Assessment (CHNA) survey to better understand the health concerns, priorities, and needs of our residents. This survey, developed in collaboration with local stakeholders, gathered community input on a range of public health topics and will help guide future programs and policies.
Key areas of focus include:
- Behavioral health and access to mental health services
- Substance use prevention and support
- Public health and emergency preparedness
- Access to healthcare and community resources
The results of the 2024 CHNA survey will serve as a foundation for Concord’s Community Health Improvement Plan (CHIP) to ensure that health initiatives align with resident needs.
